School Clinic

Overview

Our mission is to provide direct nursing services to students and staff members to maximize health and wellness in the school community. All duties are performed in accordance with district/state board of education policies and procedures and state law regarding nurse practice.

The role of the clinic staff is not to diagnose diseases or illnesses, but to report signs and symptoms of health problems and encourage the individuals to contact their own family health care providers for diagnosis and treatment. Please remember, the school clinic is not meant to be used in place of your family health care provider. 

 

Back-to-School Information

The necessary medical forms required for the start of school will be available through Awty's electronic health record system, Magnus Health Portal.

 

School Entrance Requirements for Vaccinations:

The Texas Department of Health Title 25 Health Services, §97.61-97.72 of the Texas Administrative Code requires that your child must be in compliance with the immunization schedule from the State of Texas before the start of school.

 

Proof of all immunizations must be on file at the clinic on the first day of school. Students without current proof of immunizations on file your child will not be allowed to start school.

School Entrance Requirements Medical Record Form:

The Texas Department of Health Title 25 Health Services, §37.23 of the Texas Administrative Code requires every student attending school in the State of Texas be screened upon entering the grades listed below. Each of these grades must submit a completed Medical Record Form through Magnus Health.

 

Vision and Hearing Screening is required for all new students and returning students entering the following grades: PK4 (MS), K (GS), 1st (CP), 3rd (CE2), 5th (CM2), 7th (5ème) and 9th (3ème).

 

Diabetes Screening is required for all new students and returning students entering the following grades: 3rd (CE2), 5th (CM2) and 7th (5ème).

 

Spinal Screening is required for all new students and returning students entering the following grades 6th (6ème), and 9th (3ème).


 

School Entrance Requirements For Medical Conditions:

All students requiring treatment for any chronic condition(s) including food/insect allergies, asthma, seizures, diabetes or ADD/ADDHA must supply the clinic with all medications, supplies and the supporting “Action Plans” before the start of school. Failure to comply will result in your child not being allowed to start school. This plan should be submitted to the school through Magnus Health.

 

Medication Administration

If your child requires medication administration while at school, please complete the Medical Administration information on Magnus Health.  Please note students are not allowed to carry medications on their possession therefore medications must be must be brought directly to the clinic.
