School Clinic

  

Overview

Our mission is to provide direct nursing services to students and staff members to maximize health and wellness in the school community. All duties are performed in accordance with district/state board of education policies and procedures and state law regarding nurse practice.

The role of the clinic staff is not to diagnose diseases or illnesses, but to report signs and symptoms of health problems and encourage the individuals to contact their own family health care providers for diagnosis and treatment. Please remember, the school clinic is not meant to be used in place of your family health care provider.

 

Back-to-School Information

The necessary medical forms required for the start of school will be available online and no longer be mailed out to individual families. These documents are available at the bottom of this page.

School Entrance Requirements 2010-2011

The Texas Department of Health Title 25 Health Services, §§97.61-97.72 of the Texas Administrative Code requires that your child must be in compliance with the State's current immunization laws before the start of school. The clinic will contact each family informing them of the necessary immunization(s) required for their child.

Emergency Medical Release If you have not yet completed the Emergency Medical Release with your re-enrollment please do so online. This information once submitted will arrive directly to the clinic computer for processing. This information must be completed yearly and without this information students may not leave the campus for field trips or school sponsored events.

Medical Record The Vision and Hearing Screening Program, Chapter 36 of the Health and Safety Code requires that all children enrolled in private and public schools in Texas must complete the following grade criteria screenings.

  • Vision and Hearing Screening Is required for all students entering the following grades: PK-4 (Petite Sect.), Kinder (Grande Sect.), 1st (CP), 3rd (CE2), 5th (CM2) 7th (5ème)
  • Diabetes Screening (visual inspection) Is required for all students entering the following grades: 1st (CP), 3rd (CE2), 5th (CM2) 7th (5ème).
  • Spinal Screening (visual inspection) Is required for all students entering the following grades 6th (6èMe), and 9th (3ème).

Students will not be allowed to attend class on the first day of school without up to date immunizations and a completed Emergency Medical Release on file in the clinic. These forms must be completed yearly.

Clinic Requirements

All medications to be given at the school require a note of permission Administration_of_Medicine_Authorization from the parent/guardian. Students requiring inhalers are not allowed to carry them unless their physician's order states they are allowed to do so. An Asthma_Action_Plan must be on file in the clinic. Students with food allergies requiring the use of an Epi-pen or Benadryl must have a Food_Allergy_Action_Plan on file in the clinic prior to the start of school. Students with diabetes are required to have an Individualized Action Plan on file with the clinic. The parent/guardian is responsible for providing all the necessary testing supplies and medications.

Accidents and sudden illness do occur, and having current parental information is essential in providing care for your child. Please update your child's Emergency Medical Release for Students as information changes to the clinic as soon as possible.

All new students three years and older entering grades PK-3 (Petite Sect.) through 12 (Terminale) and returning students to grades PK-4 (Mayenne Sect.), Kindergarten (Grande Sect.), 1st (CP), 3rd (CE2), 5th (CM2), 6th (6ème), and 9th (3ème) must complete the 2010-2011 Medical Record form and have it on file in the clinic.  Screenings for PK-3 should be attempted by your physician. 

All Middle and Upper School students participating in athletics must have an Athletic_Participation_Form on file with the Athletic Trainer. This requires a yearly physical prior to practice or competing in any sport(s).
